Previously the HTML files generated from the DocBook sources were named ch01.html, ch02.html, apa.html, apb.html, etc. with running numbering. This means that if one added a new chapter between existing chapters, the following chapter file names would change and break all web links pointing to them. Instead, let's use the chapter ID as the file name. This way, web links will not be broken when new chapters are added.
