Peter Meerwald a388b909f3 remap: Add stereo to mono and 4-channel special case remapping 
The generic matrix remapping is rather inefficient; special-case code
improves performance by 3x easily.

v4: split into s16 and float code, 4-channel remapping
v3: fix remap_mono_to_stereo_c(), use assignment
v2: use consistent array addressing

on Intel Core i7-870 @ 2.93 GHz (GCC 4.6, 64-bit):

HACKING:
	In order to run pulseaudio from the build dir __OPTIMIZE__ should be
	disabled (look at src/pulsecore/core-util.h::pa_run_from_build_tree()),
	this can be done by passing "CFLAGS=-O0" to the configure script:
	  ./autogen.sh
	  CFLAGS="-ggdb3 -O0" LDFLAGS="-ggdb3" ./configure
	  make
	  ./src/pulseaudio -n -F src/default.pa -p $(pwd)/src/
