efbfe5fecb core: Make debugging a bit simpler
This takes out a bunch of commented debug prints and puts them in
defines. Makes it easier to turn them all on/off at a single point.

666261ece8 memblockq: Improve debuggability by storing a name and a sample spec.
These are not used for anything at this point, but this
makes it easy to add ad-hoc debug prints that show the
memblockq name and to convert between bytes and usecs.

66cfa72eb4 native: rework handling of seeks that depend on variables the client does not know anything about
All seeks/flushes that depend on the playback buffer read pointer cannot
be accounted for properly in the client since it does not know the
actual read pointer. Due to that the clients do not account for it at
all. We need do the same on the server side. And we did, but a little
bit too extreme. While we properly have not applied the changes to the
"request" counter we still do have to apply it to the "missing" counter.
This patch fixes that.

699233fb47 native: fix request counter miscalculations
Do not subtract bytes the client sends us beyond what we requested from
our missing bytes counter.

This was mostly a thinko that caused servers asking for too little data
when the client initially sent more data than requested, because that
data sent too much was accounted for twice.

This commit fixes this miscalculation.

e61728e67a Make sure we don't get stuck when prebuf is too high
If prebuf is greater than tlength minus minreq we might end up waiting
for the buffer to fill up further however without ever asking for more
data from the client since less minreq bytes might be missing.

This fixes bug #440

d210ebbb09 rework memory block management to be thread-safe and mostly lock-free.
pa_memblock is now an opaque structure. Access to its fields is now done
through various accessor functions in a thread-safe manner.

pa_memblock_acquire() and pa_memblock_release() are now used to access the
attached audio data. Why? To allow safe manipulation of the memory pointer
maintained by the memory block. Internally _acquire() and _release() maintain a
reference counter. Please do not confuse this reference counter whith the one
maintained by pa_memblock_ref()/_unref()!

As a side effect this patch removes all direct usages of AO_t and replaces it
with pa_atomic_xxx based code.

This stuff needs some serious testing love. Especially if threads are actively
used.

0e436a6926 Rework memory management to allow shared memory data transfer. The central idea
is to allocate all audio memory blocks from a per-process memory pool which is
available as read-only SHM segment to other local processes. Then, instead of
writing the actual audio data to the socket just write references to this
shared memory pool.

To work optimally all memory blocks should now be of type PA_MEMBLOCK_POOL or
PA_MEMBLOCK_POOL_EXTERNAL. The function pa_memblock_new() now generates memory
blocks of this type by default.
