update FSF addresses to FSF web page
...
FSF addresses used in PA sources are no longer valid and rpmlint
generates numerous warnings during packaging because of this.
This patch changes all FSF addresses to FSF web page according to
the GPL how-to: https://www.gnu.org/licenses/gpl-howto.en.html
Done automatically by sed-ing through sources.

once: Fix race causing pa_once to sometimes run twice
...
There was a race in the existing code that could cause the pa_once code
to be run twice, see:
http://lists.freedesktop.org/archives/pulseaudio-discuss/2012-April/013354.html
Therefore the existing implementation was rewritten to instead look like
the reference implementation here:

rework pa_once once again, because the once function needs to have terminated before pa_once returns, regardless whether the local call executes it or another thread does.
...
With the previous code it might happen that an long-running initializing in a once function is not terminated yet when another thread thinks it already is.
