bluetooth: Avoid PA_BLUETOOTH_TRANSPORT_HOOK_REMOVED

The hook is now deprecated so avoid using it and instead use the
recently introduced PA_BLUETOOTH_HOOK_TRANSPORT_STATE_CHANGED which also
reports the disconnection event.
