Count the params as we add them to the param arrays and use that to
update the stream params instead of using hardcoded indexes and sizes.
This makes it easier to add params and it also revealed a miscounted
param.
Make a method to get the current time to compare agains the pw_time-now
field. This is currently CLOCK_MONOTONIC but make this into a method
so that we can more easily change it later.
Make the alignment parameter optional when negotiating buffers.
Default to a 16 bytes alignment and adjust for the max cpu
alignment.
Remove the useless align buffer parameter in plugins, we always
set it to 16 anyway.
When the node is driving, we need to call trigger_process()
otherwise we don't and we simply need to wait for the process() to
be called, triggered by the driver node.

SPA_MEMBER is misleading, all we're doing here is pointer+offset and a
type-casting the result. Rename to SPA_PTROFF which is more expressive (and
has the same number of characters so we don't need to re-indent).
