terminfo: add kxIN + kxOUT (focus in/out events)

These capabilities are not included in the standard ‘xterm’ or
‘xterm-256color’ terminfos. They’re used in

‘xterm+focus’ ->
‘xterm+sm+1002’ ->
‘xterm-1002|xterm+sm+1003’ ->
‘xterm-1003’

However, as far as I can tell, ncurses doesn’t use these capabilities
at all.
