Shuffle the code to avoid special conditions using the plugin type
in the generic plugin code. The rate plugin has the own
may_wait_for_avail_min callback implementation now.

We've had a few home brew atomic operations in a couple of places in
the PCM code. This was for supporting the concurrent accesses, but in
practice, it couldn't cover the race properly by itself alone.
Since we have a wider concurrency protection via mutex now, we can get
rid of these atomic codes, which worsens the portability
significantly.

When forwarding or rewinding, the frame counter was not updated. This
could result in corrupted channel status words or misplaced Z-type
preamble.

plug->client_frames and plug->slave_frames are not used by
any plugin, remove dead code.

Rename the local functions to snd1_* so that they won't be exported
out of alsa-lib.
Some functions are still kept because aserver requires them. Sigh.
- added link/unlink/link_fd fast_ops callbacks
- moved code from snd_pcm_link to pcm_hw.c
- moved "empty" routines pointing to slave to pcm_generic.c
- introduced snd_pcm_generic_t
- added poll_events to main pcm structure
- added poll_revents callback to pcm->ops structure
- fixed snd_pcm_wait() - call revents function and enhanced
error code reporting
DMIX plugin:
- more complete code (works at least for one instance)
- still no "mix" code (it's silent)
Added undo callbacks for snd_pcm_plugin based plugins.
- helpers when mmap_commit proceed only a partial transfer
Fixes to avail_update implementation in pcm_hw.c.
