Bug fixed:
- duplicated deallocation for mmap'ed area
- missing deallocation in es18xx
- some bugs in volbal conversion

New features:
- virtual mixer for card-share (it seems to work, but I feel that it
  need further testing)

Misc:
- reorganization and extension of rate.c

Jaroslav changes:
- volbal source is mostly reverted back (no card-share mixer polling)
